Chronic condition management
People diagnosed with a chronic condition, such as diabetes or asthma, do better when they follow evidence-based standards of care for their illness.
Our nurses analyze your employer’s medical and pharmaceutical claims data to see if you have been treated for any of nine common chronic conditions. For those who have one of these conditions, we will look at your claims data to see if you are receiving medical treatment consistent with evidence-based standards of care. If we identify a potential gap in your care, such as a missed annual blood test, we will send a reminder for you and your doctor, alerting you both to a possible missed opportunity to keep your condition in check.
YourCare identifies members with these nine conditions:
- Asthma
- Chronic musculoskeletal pain (back pain, neck pain & osteoarthritis)
- Chronic obstructive pulmonary disease
- Congestive heart failure
- Coronary artery disease
- Depression
- Diabetes
- High blood pressure
- High cholesterol
