YourCare 2012 Incentive Program
Health eRewards
An incentive program from YourCare offers a chance for you to get healthy and make some money! By completing the Health eRewards program, you can qualify for monthly $500 prizes throughout the year. The program runs Jan. 1, 2012 through Dec. 31, 2012.
To Qualify
You must be eligible on the medical plan at the time of the prize drawing, be a YourCare member (employee or spouse), be over the age of 17, and complete your Health Risk Assessment (HRA) in order to qualify for a prize. Winners will be determined at the end of each month.
The Prize
Once members complete their HRA, they will be entered into a monthly drawing for $500. All eligible participants who do not win will continue to be entered into every monthly drawing through the close of the program on December 31, 2012. Winners will be drawn across all YourCare members. If you are a winner, we will mail you a $500 VISA gift card the following month. Once a participant wins, he or she will not be eligible for other drawings. You must be eligible to win all prizes.
